What you will need:

1. U.S. Passport – You must provide your signed passport. Your passport must be valid for at least 6 months and have a blank page available.
2. Philippines Visa Application Form – The completed visa application form must be signed and notarized; click here for a .pdf of the application. Because the application changes on occasion, we’ve also provided a link to the Philipines Embassy website so you may get the most recent version directly from the Embassy website. The application form must be filled out completely and have answers for all fields. It must display your full name as appears in your passport, and it must be signed and notarized.
3. Washington Express Visas Visa Request Form – filled out completely.
4. Photographs – Please provide 2 (two) color, passport size photos. The photographs must be printed on photo paper, must be 2″ by 2″ and have a white background, and must have been taken in the last 6 months.
5. Proof of sufficient funds – a copy of your recent bank statement or a letter from your bank.
6. Invitation letter – An invitation letter from your host stating your relationship, purpose of travel, length of stay, accommodations and contact information.
7. Copy of your airline ticket or travel agent itinerary.

Vaccination against Yellow Fever required, if arriving within 6 days after leaving or transiting countries with infected areas.

Exempt from Yellow Fever vaccination:

– Children under 1 year of age who are, however, subject to isolation or surveillance when indicated.

– Transit passengers not leaving the transit lounge of the airport.
– Those not leaving the airport in the infected countries concerned.
Recommended:
– Malaria prophylaxis. Malaria risk exists throughout the year in areas below 600 m, except in the provinces of Aklan, Bilaran, Bohol, Camiguin, Capiz, Catanduanes, Cebu, Guimaras, Iloilo, Leyte, Masbate, northern Samar, Sequijor and metropolitan Manila. No risk considered to exist in urban areas or in the plains. P. falciparum resistant to Chloroquine and sulfadoxine-pyrimethamine reported. Recommended prevention in risk areas: IV.
