All packages mailed to Washington Express Visas must include the Passport Order Form, 2 copies of the Washington Express Visas Passport Authorization Form completed by hand, and the remaining required documents suitable for your passport application.

Unfortunately, the passport office no longer offers adding pages as a service. Traveler’s who no longer have visa pages available must renew their passport. The passport office suggests that frequent travelers should opt for the 52-page book offered at the same price as the traditional 28-page book.

 

Minors must appear with their parents in front of an acceptance agency to confirm the minors identity. If only one parent can appear in front of the acceptance agency, then the absent parent must complete the Statement of Consent form, sign it, and have the document notarized accompanied with the remaining documents. You’re passport photo must be: 

  • In color.
  • 2 x 2 inches in size.
  • Printed on thin, photo-quality paper.
  • Taken within the past 6 months, showing current appearance.
  • Full face, front view with a plain white or off-white background.
  • Between 1 inch and 1 3/8 inches from the bottom of the chin to the top of the head.
  • Taken in normal street attire: Uniforms should not be worn in photographs except religious attire that is worn daily.
  • Do not wear a hat or headgear that obscures the hair or hair line.
  • If you normally wear a hearing device, wig or similar articles, they should be worn for your picture.
  • Starting November 1, 2016, eye glasses will no longer be allowed in passport photos. If you must wear eye glasses for medical reasons, you’ll need to obtain and submit a signed statement with your passport application from a medical professional or health practitioner.
  • Articles worn for religious or medical reasons should be accompanied by a statement to that effect.
