• Azerbaijan Visa Application Form – Must be filled out online. It must also be printed and signed, along with a copy.  The online Azerbaijan visa application can be confusing.  Please note:
  • Each applicant will need a unique, valid email address.
  • The CAPTCHA (human validation) may require multiple attempts.  You may need to refresh as many times as needed to pass the validation, so be patient.
  • There is not a signature line on the printed form, so please sign and date at the bottom of the last page.
  • If you are unable to submit the application, it may be that you don’t have the latest version of the supported browsers.
• Washington Express Visas Visa Request Form – filled out completely.
• Photographs – Please provide two color, passport size photos.  The photographs must be printed on photo paper,  must be 2″ by 2″ and have a white background, and must have been taken in the last six months.
• Tourist voucher/Official Invitation – an official voucher from an authorized travel organization or hotel in Azerbaijan, registered through the Consular Department of the Ministry of Foreign Affairs in Azerbaijan, or hotel confirmation or  letter from travel agent and round-trip or onward flight itinerary.
• Proof of departure – Copy of your airline ticket or travel agent itinerary.
• All applications now require approval from Baku, which can extend the processing time.

3. Washington Express Visas Visa Request Form – filled out completely.

5. Official Invitation – Inviting party should send an invitation through the Consular Department of the Ministry of Foreign Affairs of Azerbaijan.
6. A business letter of introduction from your company (on company letterhead) addressed to the Embassy of Azerbaijan clearly stating the purpose of travel, the name and contact of the company in Afghanistan you’re visiting, and a financial guarantee for all of your expenses.  Click here to see a sample business letter.
7. Copy of your airline ticket or travel agent itinerary.
8. All applications now require approval from Baku, which can extend the processing time.

• Warning: Limited risk of malaria exclusively in the benign (P.Vivax) form from June to October in lowland areas, mainly between the Kura and Arax rivers.  Recommended prevention: II.

• Passport required.  Passport and/or passport replacing documents must be valid for the period of intended stay.
• Visa required.
• Visa Issuance: Holders of an official invitation letter issued by the State Migration Service of Azerbaijan (or other Azerbaijan government authority) can obtain a visa on arrival.
• Staff of the Asian Development Bank (ADB) and their dependents can obtain a visa on arrival if holding an official invitation letter from the ADB.
