What you will need: 

  1. Washington Express Visas Visa Request Form – filled out completely.
  2. U.S. Passport – You must provide your signed passport. Your passport must be valid for at least 6 months and have two adjacent blank pages available.
  3. Bolivia Visa Application Form – Complete the Sworn Statement and Visa Application available online at Bolivia Embassy Website. *Please, upload electronic copies/scans of all the requested documents to your application when you apply.
  4. Photograph – Please provide two color, passport size photos. The photograph must be printed on photo paper, it must be 2″ by 2″ and have a white background, and it must have been taken in the last 6 months.
  5. Proof of legal stay or residence status (applicable to those not applying for the visa in their country of citizenship). For example a notarized copy of Green Card.
  6. Personal invitation – if visiting friends or family, you must provide a letter of invitation from your host in Bolivia.  The letter must contain personal information of the applicant, details of the visit, and details of the inviting party.
  7. Copy of your airline ticket or travel agent itinerary showing your name as well as travel information.
  8. Copy of hotel reservations (Applicant’s name must be on hotel reservation).
  9. Yellow Fever Vaccination – Copy of International Certificate of Vaccination for Yellow Fever.
  10. Bank statement –  Copy of the applicant’s most recent monthly bank statement. The statement must clearly show the applicant’s name as the account holder, the balances of the accounts, and the date of the statement

Former citizens of Bolivia who have acquired U.S. citizenship are exempt from the visa requirement, provided evidence of their condition of Bolivians with any of the following current or expired documents:

      • Bolivian Identity card, birth certificate, family book, Military Service Book, Bolivian Passport, Bolivian National Single Registry (RUN). Even a United States passport alone that states that the Place of Birth is BOLIVIA will suffice.

Business letter of introduction from your company (on company letterhead) addressed to the Embassy of Bolivia stating the purpose of travel, the name and contact of the company in Bolivia you’re visiting, and a financial guarantee for all of your expenses. Click here to see a sample business letter.  The letter and your visa application must specify the type of visa(single, multiple entry), and duration(6 months, 1 year) desired.
  7. Letter of invitation from the company/entity in Bolivia.
  8. Copy of your airline ticket or travel agent itinerary showing your name as well as travel information.
  9. Police Report – Police report or Certificate of Good Conduct from applicant’s local police station.
  10. Yellow Fever Vaccination – Copy of International Certificate of Vaccination for Yellow Fever.
  11. Bank statement –  Copy of the applicant’s most recent monthly bank statement. The statement must clearly show the applicant’s name as the account holder, the balances of the accounts, and the date of the statement.

No vaccinations are required to enter Bolivia from any country.

Recommended:

  • Malaria prophylaxis. Malaria risk – including the malignant (P. Falciparum) form – occurs in Hainan and Yunnan. Chloroquine and sulfadoxine-pyrimethamine resistant P. falciparum reported. Risk of P. Vivax malaria exists in Fujian, Guangdong, Guangxi, Guizhou, Hainan, Sichuan, Xizang (only along the valley of the Zangbo river in the extreme south-east) and Yunnan. There is generally very low malaria risk (P. vivax only) in Anhui, Hubei, Hunan, Jiangsu, Jiangxi and Shandong, although within these provinces this risk may be higher in areas of local outbreak. Where transmission exists, it occurs below 1500 m: from July to November north of latitude 33 degrees north, from May through December between 33 and 25 degrees north, and throughout the year south of 25 degrees north. There is no malaria risk in the urban areas or densely populated plain areas. In general, tourists do not need to take malaria prophylaxis unless they plan to stay in remote rural areas in the provinces listed above. Recommended prevention in risk areas:II ; in Hainan and Yunnan: IV
  • Passport and/or passport replacing documents must be valid on arrival.

Additional Information:

  • Note: different passport validity may be required when applying for a visa.
  • Valid visas or resident permits in expired passports are accepted, provided holding valid passport of the same nationality.
  • Travel on a one-way ticket is accepted. Visitors must hold sufficient funds to cover their stay and all documents required for their next destination.

Warning:

  • Passengers not complying with the entry or transit regulations will be deported by same or returning aircraft.
